How Florida Blue and BCBS Claims Behave

BlueCross BlueShield operates as independent plans, so a member's home plan governs benefits even when care is delivered in Florida. Local claims run through Florida Blue while out-of-area members are processed through the BlueCard arrangement.

  • Member home plan identified before benefits are assumed
  • Local Florida Blue and out-of-area BlueCard claims handled separately
  • Plan product confirmed since networks differ within the same carrier
  • Portal used for eligibility, authorization and claim status

BlueCross BlueShield Billing Services Explained

BlueCross BlueShield in Florida operates as Florida Blue, an independent licensee of the Blue Cross Blue Shield Association. Florida Blue is the state's largest commercial payer, covering a substantial portion of employer-sponsored and individual health plan members. Florida Blue billing requires navigation of BlueOptions, BlueCare, and BlueSelect network tiers, plan-specific prior authorization rules, and Florida Blue's medical policies that define clinical coverage criteria.

Medsure RCS manages Florida Blue billing with accurate plan identification, prior authorization submission through the Availity portal, and denial management for Florida Blue administrative and medical necessity denials. We track Florida Blue policy updates and apply current medical policies to every applicable claim before submission. Our Florida Blue billing service includes provider credentialing for all BCBS network tiers.

How It Works: Step by Step

Step1

Florida Blue Plan Tier Verification

The patient's specific Florida Blue plan and network tier is confirmed, BlueOptions, BlueCare, BlueSelect, or FEP, since each tier has different in-network providers, benefits, and authorization requirements.

Step2

Availity Prior Authorization Submission

Prior authorization requests are submitted through the Availity portal with clinical documentation matching Florida Blue medical policy criteria for the applicable service.

Step3

Florida Blue Claims Submission

Claims are submitted electronically through Availity with Florida Blue's payer IDs assigned correctly by plan type, including separate routing for FEP (Federal Employee Program) plans.

Step4

Florida Blue Denial Management

Denials are reviewed by reason code and appealed within Florida Blue's filing windows through the Availity appeals workflow with clinical policy documentation.

Common Mistakes in BlueCross BlueShield Billing Services

Mixing BlueOptions and BlueSelect Network Rules

Florida Blue operates multiple networks with different in-network providers and benefit levels. BlueOptions is a broad network; BlueSelect is a tiered performance network with different cost-sharing. Applying the wrong network rules for a patient's specific plan results in billing errors and patient balance disputes.

Billing FEP as Commercial Florida Blue

Federal Employee Program (FEP) plans are administered by Florida Blue but operate under a separate national program with different payer IDs, benefit structures, and claims processing rules. Submitting FEP claims with commercial Florida Blue payer IDs results in rejection.

Missing Florida Blue Medical Policy Criteria

Florida Blue medical policies require specific ICD-10 diagnoses and clinical documentation for coverage of imaging, specialty procedures, and DME. Missing the specific covered diagnosis codes listed in the applicable medical policy results in denial.

Not Checking Florida Blue Pre-Certification for Inpatient Services

Florida Blue requires pre-certification for inpatient admissions, outpatient surgeries, and certain diagnostic procedures. Services provided without pre-certification receive administrative denial that may not be reversible on appeal.

BlueCross BlueShield Billing Services, Frequently Asked Questions

These are Florida Blue's different network tiers. BlueOptions is a broad preferred provider network. BlueSelect is a tiered performance network where providers are ranked by cost and quality, with different patient cost-sharing by tier. BlueCare is the HMO product requiring a primary care physician and referrals. Each has different in-network providers and benefit levels.

Federal Employee Program plans are BCBS Association programs covering federal government employees. They use a national payer ID (00910) rather than Florida Blue's commercial payer ID, follow different benefit schedules, and are processed through a separate national claims system. FEP claims must be routed separately from commercial Florida Blue claims.

It depends on the plan type. BlueCare (HMO) requires a referral from the PCP for specialist visits. BlueOptions and BlueSelect PPO plans generally do not require referrals, but may require prior authorization for certain specialist services, imaging studies, and procedures. We verify authorization requirements at eligibility for every visit type.

For employer groups with employees on multiple Florida Blue plans, we verify the specific plan enrollment for each patient at every visit, since employees at the same employer may be on different plan tiers. We route claims to the correct plan and apply plan-specific authorization and fee schedule rules per patient.
