Billing Modifier

Modifier GA: ABN Required Waiver of Liability

Modifier GA indicates a required Advance Beneficiary Notice was issued before a potentially non-covered Medicare service.

Quick Summary

When to Append Modifier GA

Append to a service code when an Advance Beneficiary Notice (ABN) has been issued to the Medicare patient because the provider expects Medicare to deny the service as not medically necessary.

  • A valid, signed ABN form (CMS-R-131) must be on file before the service is rendered.
  • The ABN must describe the specific service and estimated cost to the patient.
  • Modifier GA is a Medicare-specific modifier.
  • If a service is denied for medical necessity and no ABN was collected, the provider cannot bill the patient.
  • Modifier GA protects the provider's right to collect patient payment.

Modifier GA in Detail

What does Modifier GA cover in detail?

Modifier GA indicates that an Advance Beneficiary Notice was issued to the patient before a service that Medicare may determine is not covered, protecting the provider's right to bill the patient if Medicare billing denies the claim.

ABN documentation must be on file and the service must qualify; all GA modifier requirements are covered in the billing modifiers reference. Where no notice was obtained the claim carries modifier GZ instead.

Modifier GA is one of three Medicare liability modifiers, and choosing the wrong one shifts who pays. GA is appended when a signed Advance Beneficiary Notice (form CMS-R-131) is on file for a service the provider expects Medicare to deny as not reasonable and necessary, which preserves the right to bill the patient. GZ is used when the provider expects denial but did NOT obtain an ABN, so the charge becomes provider liability and cannot be billed to the patient. GY is used for services that are statutorily excluded from Medicare altogether, where an ABN is not required because the item was never a Medicare benefit. Appending GZ where GA belongs is a common and expensive error, because it writes off revenue the practice was entitled to collect.
