Pediatric Billing Services
Pediatric billing services for well-child visits, immunization billing, preventive care, and acute pediatric encounters.
What Pediatric Billing Has to Handle
Pediatrics runs high volume at low per-claim value, with a large share of Medicaid managed care. Preventive visits, immunizations and administration are billed together but under separate rules, and administration is commonly the part that gets missed.
- Preventive visits and immunizations billed under their own rules
- Administration captured alongside each vaccine given
- Florida Medicaid managed care plan requirements applied
- Sick visits reported separately from preventive care when documented
Pediatric Billing Services Explained
Pediatric billing centers on well-child visits with preventive care coding, immunization administration billing, and Medicaid billing under Florida KidCare and the EPSDT program. Preventive care codes are age-banded, and immunization administration codes require separate billing for each vaccine administered with add-on codes for counseling. Paediatric patients receiving behavioural therapy are billed under ABA therapy billing rules.
Medsure RCS manages pediatric medical billing services for Florida practices with expertise in well-child visit coding, eligibility verification for KidCare and Medicaid plans, and the documentation requirements for developmental screening, EPSDT screening, and vaccine administration. We handle both commercial and Medicaid pediatric billing across all visit types. Pediatrics is part of our specialty medical billing program, with deep experience in Florida Medicaid billing for pediatric practices.

How It Works: Step by Step
Visit Type Classification
Each encounter is classified as preventive (well-child) or problem-based (sick visit), with the correct code type applied for each. Same-day preventive and sick visits require modifier 25.
Age-Banded Code Selection
Preventive care codes (99381-99395) are selected by the patient's exact age at the time of service, not their scheduled age group.
Immunization Administration Billing
Each vaccine is billed separately with the vaccine product code (90655-90749) and the administration code (90460-90461 per antigen administered when counseling is provided).
Medicaid and EPSDT Compliance
Florida KidCare and Medicaid claims are submitted with EPSDT screening documentation, developmental screening results, and any referrals generated from the well-child visit.
Why This Matters for Your Revenue Cycle
Every vaccine is billed with the correct product code and administration code per antigen administered.
Pediatric-specific age banding and preventive vs problem-based separation drive first-pass acceptance above 95%.
Florida Medicaid EPSDT screening requirements are met and documented on every applicable well-child claim.
Pediatric claims are submitted within 48 hours of encounter documentation receipt.
Common Mistakes in Pediatric Billing Services
Wrong Age Bracket for Preventive Code
Preventive care codes are tiered by patient age at the date of service. Using the code for the wrong age bracket based on a scheduled age range rather than actual visit date age is a documentation and billing accuracy error.
Missing Modifier 25 for Same-Day Sick Visit
When a medically significant sick visit is performed during a well-child visit, both the preventive code and the appropriate E/M code are billable with modifier 25. Without modifier 25, the E/M service is bundled into the preventive code and lost.
Immunization Administration Code Errors
Immunization administration under age 6 with counseling uses 90460 for the first injection and 90461 for each additional component. Without counseling, 90471 and 90472 apply. Mixing these code sets or failing to bill each antigen separately results in systematic underpayment.
Medicaid Non-Covered Provider Billing
Florida Medicaid covers specific provider types for pediatric services. Billing under the correct rendering provider NPI and verifying provider enrollment status before billing prevents coverage-based denials.

Pediatric Billing Services, Frequently Asked Questions
Each vaccine is billed separately using the vaccine product code for that specific immunization. The administration is billed with 90460 for the first injection and 90461 for each additional antigen component when physician counseling is provided, or 90471 and 90472 without counseling. A visit with 5 vaccines might have 5 separate vaccine codes and 5 separate administration codes.
Yes, with modifier 25 on the E/M code for the separately identifiable sick visit. The documentation must clearly show that the E/M for the acute problem was separate from and beyond the routine preventive service. Both codes are billed: the preventive code (no modifier) and the E/M code with modifier 25.
Yes. Florida KidCare includes multiple managed care plans (Healthy Kids, MediKids, Florida Healthy Kids) with different billing requirements and covered service lists. We verify each child's specific plan enrollment and submit claims to the correct plan with applicable covered service codes.
EPSDT (Early and Periodic Screening, Diagnostic, and Treatment) is a Medicaid mandate that requires comprehensive preventive health services for children under 21. Florida Medicaid covers EPSDT screenings and requires specific screening codes and documentation. We ensure EPSDT-eligible claims include the required screening documentation and codes to maximize covered benefits.
